340B Has Never Been More Important. Or More Complicated.

By Nick Gnadt, PharmD, 340B ACE, Director of Pharmacy Professional Services, SpendMend

For more than 30 years, the 340B Drug Pricing Program has played a vital role in helping safety-net providers stretch scarce resources and continue serving the patients and communities that depend on them.

That mission has not changed.

But almost everything around it seems to be changing.

For pharmacy leaders, managing a successful 340B program has always required vigilance. Covered entities need to remain HRSA-ready; maintain program integrity; understand an evolving regulatory environment; and make sure they are maximizing the value of a program that can be critical to their organization.

But today, those responsibilities are being layered with an extraordinary amount of change.

Manufacturer actions. New legislation and regulation. CMS reimbursement changes. Medicare Maximum Fair Price (MFP). The proposed 340B rebate model. And almost certainly more changes we haven’t seen yet.

In other words, 340B has rarely been more important. And it may never have been more complicated.

From discounts to data, rebates, and reconciliation

For years, much of the financial side of 340B was centered on drug acquisition cost: Is this an eligible 340B transaction? Are we purchasing the drug at the appropriate price? Are we operating the program compliantly?

Those questions remain.

But increasingly, pharmacy leaders also need to think about data, reimbursement, rebates, reconciliation, and ultimately, margin.

MFP is one example. The proposed 340B rebate model is another.

While the mechanics are different, both illustrate the increasingly complicated financial environment surrounding pharmacy. It is no longer enough to know what a drug should cost. Organizations increasingly need the data and processes to validate transactions, understand reimbursement, reconcile payments and make sure the economics ultimately work as intended.

The proposed 340B rebate model may make that challenge particularly visible. Instead of simply receiving the 340B discount upfront, participating covered entities could purchase affected drugs at a higher acquisition price and then need to rely on accurate, timely data submission and subsequent rebates to bring the net drug cost back down.

That creates new questions.

Is our data clean? Are we capturing every eligible transaction? Are submissions occurring on time? Are we receiving the rebates we expect? Are we reconciling those payments correctly? And what happens when something doesn’t match?

Those aren’t theoretical questions – they directly affect the financial performance of the pharmacy and potentially the resources available to support the organization’s mission.

The bigger challenge is what comes next

MFP and the rebate model are important. But I don’t believe pharmacy leaders should look at either of them in isolation.

The larger story is that 340B itself is entering a period of extraordinary change.

And if SpendMend has learned anything from working with hundreds of covered entities and conducting thousands of 340B audits, it’s that successful programs aren’t built around reacting to the latest regulation.

They’re built around readiness.

That means having the right people, processes, technology, data and expertise in place to maintain compliance today while being prepared to respond to whatever happens tomorrow.

At SpendMend, that’s increasingly how we think about our role.

We don’t see 340B as a collection of individual services. An audit over here. Consulting over there. Technology somewhere else. MFP reconciliation today and potentially rebate reconciliation tomorrow.

SpendMend looks at the entire program.

From planning and implementation to compliance and audit readiness. From education and ongoing program support to optimization and financial performance. And now, increasingly helping covered entities navigate a world of rebates, reimbursement, and reconciliation.

Because the individual challenge may change – the need for a trusted partner does not.

For now and for whatever comes next

Nobody can predict exactly what the 340B landscape will look like several years from now. At the moment, predicting what it will look like several months from now can be challenging enough.

But covered entities shouldn’t have to navigate that uncertainty alone.

Our goal at SpendMend is to be your end-to-end 340B program partner, helping you maintain a strong, compliant program today while preparing for what tomorrow brings.

Because whether the next challenge involves HRSA compliance, MFP, a rebate model, reimbursement changes, manufacturer requirements or something none of us is talking about yet, the objective remains the same:

Protect the program. Protect its value. And protect your ability to serve the patients and communities for whom 340B was created.

SpendMend. Your end-to-end 340B program partner, for now and for whatever comes next.
